Normally, students progress through a degree program on the major/minor requirements track outlined in the Catalogue, but periodically a student will fulfill one or more requirements for a major or minor with a course outside that track, e.g., study away. 
 
To authorize an exception for a student with a declared major or minor and make it part of their academic record for advising, a representative from the department/program should complete this form in its entirety and submit it to the Office of the Registrar for processing. Once submitted, the exception will appear in Degree Progress within 2-3 weeks. 
 
Please complete a separate form for each student. You may enter up to four exceptions for the same student with this form.
