Degree Requirement Exception Authorization Form
Normally students progress through a degree program on the outlined track, but periodically a student will fulfill one or more requirements for a major or minor with a course outside that track, e.g. study away.
To authorize an exception and make it part of the student's academic record for advising, the authorized individual should complete this form in its entirety and submit it to the Office of the Registrar for processing. Once submitted, the exception will appear in Degree Works within 2-3 weeks.
Please complete a separate form for each student. You may enter up to four exceptions for the same student with this form. If you have any questions, please

The degree audit in Degree Works is not the student's academic transcript and it is not official notification of their completion of degree or certificate requirements.
